Black Belts: Your black belt number is on your black belt
certificate and on the gold metal card you received with it. If
you no longer have these items OR are a new black belt and have
not received these items, please contact the UFAF office with
this form to get your black belt number.
UNDER Black Belt Students and
Cross-Over Black Belts: You can get your Student
Member ID from your instructor OR you can complete
this form to
request it from the UFAF office. |
